KINDERGARTEN REGISTRATION
FOR SCHOOL YEAR 2019-2020 

A child must be 5 years old before July 01 to enter Kindergarten



Jefferson County School policy allows limited early acceptance for some children born between July 01 and July 15th if they meet eligibility requirements.

To be eligible for early Kindergarten entrance, the child must score at or above the ninety-fifth percentile on a standardized comprehensive measure of developmental skills when compared to age norms appropriate to a child who would be 5 years old prior to the July 01 deadline.  This assessment will be administered by qualified Jefferson County Schools personnel.

EVERY CHILD ENROLLING MUST HAVE A COMPREHENSIVE HEALTH SCREENING as well as proof of a DENTAL EXAMINATION COMPLETED



Health Check Forms are available at local physicians’ offices, the Jefferson County Board of Education office and the Pupil Services webpage, under Medical Forms.  There is no specific form for the dental exam – at a minimum, proof of exam should show the date of the exam (must be within the previous 12 months) and have the dentist’s signature.

REQUIRED IMMUNIZATIONS
Please be prepared to show proper documentation

 

DPT – Four doses required.  One dose must be after the 4th birthday.

Polio – Three doses required.  One dose must be after the 4th birthday.

Measles, Mumps & Rubella (MMR) – Two doses required.  The first dose must be after the 1st birthday.

Varicella (Chickenpox) – Two doses required.  The first dose must be after the 1st birthday.

Hepatitis B – Three doses required.  The last dose must be after the age of 6 months.

Tuberculin Test - Only required for students in high-risk
categories.
